bacterial meningitis

December 31, 2009

Dear Incoming Resident,
University Forest would like to take this opportunity to say thank you for choosing to live with us in 2010. In preparation for your arrival we are sending out this alert to inform all incoming residents of an important change in Texas law. The following procedure must be completed before you can move-in to University Forest.

According to Texas Higher Education Coordinating Board Rules chapter 21: Subchapter T (21.613), beginning January 1, 2010, state law requires that first-time and transfer students who plan to live in on-campus housing must show evidence of receiving the vaccination against bacterial meningitis.  Date of receipt of the vaccination must be no less than 10 days prior to move-in to campus housing.  Exceptions are defined in chapter 21: Subchapter T (21.614).

Evidence of the student receiving the vaccination from an appropriate health practitioner must be received by the administrative official at the University of Houston Clear Lake in one of the following three formats:

  • A document bearing the signature or stamp of the physician, or public health professional. (must include month, day and year the vaccination was administered)
  • An official immunization record generated from a state or local health authority. (must include month, day and year the vaccination was administered)
  • An official record received from school officials, including a record from another state. (must include month, day and year the vaccination was administered).

The university can provide the vaccination to enrolled students at a discounted rate.  For full information please contact the University of Houston Clear Lake Health Center at www.uhcl.edu/health or by phone at 281.283.2626.

University of Houston Clear Lake and University Forest will require vaccination or proof of exception before you can take occupancy.  Please know that we are looking forward to watching you build relationships, make your apartment your home and launch your future from University Forest.

Sincerely,

Shawn Floyd
